The objective of the study was to answer the question of whether the observed increase in incidence is a consequence of the recent sharp increase in cases of hepatitis C virus (HCV) transmission, using molecular genetic analysis.</p> <p><bold>Materials and methods.</bold> Amplification of HCV core/E1 fragment and Bayesian phylogenetic analysis of corresponding nucleotide sequences were performed for serum samples from 344 patients with chronic hepatitis C diagnosed in 2022–2023, which accounted for 12.6% of all chronic hepatitis C cases diagnosed during this period in Omsk Region (<italic>n</italic> = 2730).</p> <p><bold>Results.</bold> Phylogenetic analysis demonstrated the presence of regional HCV clusters formed in 1990–2000s (95% HPD 1980–2014). The majority (86.0%, 295 out of 343) of HCV genotype 1a, 1b, 2a, 2c, 2k and 3a sequences isolated in 2023–2024 belong to different lineages circulating in the region for at least 10 years, which, together with the absence of large, homogeneous clusters with a recent tMRCA suggests the absence of a recent epidemiological link between these HCV cases. However, 12 (3.5%) sequences formed pairs with divergence in 2021–2023 (95% HPD 2018–2024), which indicates recent infection and suggests the presence of an epidemiological link between these cases of infection. Such pairs were identified for HCV genotypes 1a, 1b, 2a and 3a.</p> <p><bold>Conclusion. </bold>The observed upsurge in chronic hepatitis C incidence in Omsk Region apparently reflects the accumulated number of cases of HCV infection in the region resulted from transmissions occurred 10–20 or more years ago, and is a consequence of the increase in the HCV testing coverage. With an increase in the testing rates, especially in vulnerable groups where a significant number of undetected HCV infections are concentrated, an increase in the incidence of chronic hepatitis C can be expected in other regions of the country.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> Резкий подъём регистрируемой заболеваемости хроническим гепатитом С (ХГС), превышающий более чем в 3 раза среднероссийский уровень, был отмечен в Омской области начиная с 2023 г.</p> <p><bold>Целью </bold>исследования являлся ответ с помощью молекулярно-генетического анализа на вопрос о том, является ли наблюдаемый подъём заболеваемости следствием недавнего резкого роста случаев заражения вирусом гепатита С (ВГС).</p> <p><bold>Материалы и методы.</bold> Амплификацию участка сore/E1 генома ВГС и байесовский филогенетический анализ полученных нуклеотидных последовательностей выполняли для образцов сыворотки крови от 344 пациентов с ХГС, впервые диагностированным в 2022–2023 гг., что составило 12,6% впервые выявленных в этот период в Омской области случаев ХГС (<italic>n</italic> = 2730).</p> <p><bold>Результаты.</bold> Филогенетический анализ продемонстрировал наличие региональных кластеров ВГС, сформировавшихся в 1990–2000 гг. (95% ДИ 1980–2014). Большинство — 295 (86,0%) из 343 геноизолятов ВГС генотипов 1а, 1b, 2a, 2c, 2k и 3a, выделенных в 2023–2024 гг., — принадлежат к разным линиям, циркулирующим в регионе не менее 10 лет, что в сочетании с отсутствием крупных однородных кластеров с недавним временем существования последнего общего предка позволило предполагать отсутствие недавней эпидемиологической связи между данными случаями инфекции. В то же время 12 (3,5%) последовательностей формируют пары с расхождением в 2021–2023 гг. (95% ДИ 2018–2024), что указывает на недавнее инфицирование и позволяет предполагать наличие эпидемиологической связи в данных случаях заражения. Такие пары выявлены для ВГС генотипов 1а, 1b, 2а и 3а.</p> <p><bold>Заключение.</bold> Наблюдающийся в Омской области подъём заболеваемости ХГС, по-видимому, отражает накопленное в регионе число случаев хронической ВГС-инфекции, возникших в результате заражения 10–20 и более лет назад, и является следствием увеличения охвата населения тестированием на маркеры гепатита С.
