Beijing genotype, its clusters B0/W148 and Central Asian/Russian were determined by PCR detection of the specific markers: IS<italic>6110</italic> insertions in the <italic>dnaA-dnaN</italic> region, mutations in codons 48 of the <italic>mutT4</italic> gene (CGG &gt; GGG) and 58 of the <italic>mutT2</italic> gene (GGA &gt; CGA), IS<italic>6110</italic> insertions in the Rv2664 region-Rv2665 and Rv1359-Rv1360, substitutions G &gt; A in the <italic>sigE</italic> gene. Non-Beijing strains were spoligotyped.</p> <p><bold>Results.</bold> Drug resistance was detected in 41.6% (37/89), MDR — in 33.7% of strains. In 90% (27/30) of MDR strains, resistance to rifampicin and isoniazid was due to <italic>rpoB</italic> <italic>Ser531Leu</italic> and <italic>katG</italic> <italic>Ser315Thr</italic> mutations. Following <italic>M. tuberculosis</italic> genotypes were identified: Beijing (67.4%), T (14.6%), Ural (4.5%), Haarlem (4.5%), LAM (2.3%) and CAS1-Delhi (1.1%). Among the Beijing strains, clusters Central-Asian/Russian (60%; 36/60) and B0/W148 (30%; 18/60) prevailed. The majority of MDR strains belonged to the Beijing family (93.3%; 28/30), of which 64.3% (18/28) and 21.4% (6/28) belonged to clusters B0/W148 and Central-Asian/Russian, respectively.</p> <p><bold>Conclusion.</bold> In heterogeneous population of the causative agent of tuberculosis in the Arkhangelsk region, the most common strains were those of the Beijing genotype; in 2018 its share increased to 67.4% (40.4% in 1998–1999). Among MDR strains, the proportion of Beijing reached 93.3%, of which more than half (64.3%) belonged to the epidemiologically and clinically significant in Russia cluster B0/W148.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Введение.</bold> На фоне улучшения основных эпидемиологических показателей (заболеваемость и смертность) по туберкулёзу (ТБ) в Архангельской области доля впервые выявленных больных ТБ с множественной лекарственной устойчивостью (МЛУ) возбудителя увеличилась с 18,7% в 2002 г. до 33,8% в 2018 г.</p> <p><bold>Целью</bold> исследования была генотипическая характеристика штаммов <italic>Mycobacterium tuberculosis, </italic>полученных от впервые выявленных больных ТБ в Архангельской области в 2018 г.</p> <p><bold>Материалы и методы. </bold>Изучены 89 штаммов <italic>M. tuberculosis</italic>, выделенных в 2018 г. от впервые выявленных больных ТБ. Принадлежность к генотипу Beijing, кластерам B0/W148 и Central-Asian/Russian Beijing определяли с помощью ПЦР-детекции специфических маркеров: инсерции IS<italic>6110</italic> в области <italic>dnaA-dnaN</italic>, мутаций в кодонах 48 гена <italic>mutT4</italic> (CGG&gt;GGG) и 58 гена <italic>mutT2</italic> (GGA&gt;CGA), вставки IS<italic>6110</italic> в области Rv2664-Rv2665 и Rv1359-Rv1360, замены G&gt;A в гене <italic>sigE.</italic> Штаммы non-Beijing были сполиготипированы.</p> <p><bold>Результаты. </bold>Лекарственной устойчивостью обладали 41,6% (37/89), МЛУ — 33,7%<bold> </bold>штаммов. У 90% (27/30) МЛУ-штаммов устойчивость к рифампицину и изониазиду была обусловлена мутациями <italic>rpoB</italic> <italic>Ser531Leu</italic> и <italic>katG</italic> <italic>Ser315Thr</italic>. Выявлены генотипы<italic> M. tuberculosis</italic>: Beijing (67,4%), T (14,6%), Ural (4,5%), Haarlem (4,5%), LAM (2,3%) и CAS1-Delhi (1,1%). Среди штаммов Beijing преобладали кластеры Central-Asian/Russian (60%; 36/60) и B0/W148 (30%; 18/60). Большинство МЛУ-штаммов принадлежали к семейству Beijing (93,3%; 28/30), из которых 64,3% (18/28) и 21,4% (6/28) — к кластерам B0/W148 и Central-Asian/Russian соответственно.</p> <p><bold>Заключение.</bold> В гетерогенной популяции возбудителя ТБ Архангельской области наиболее распространёнными были штаммы генотипа Beijing, причём в 2018 г. его доля увеличилась до 67,4% (в 1998–1999 гг. — 40,4%).
